Tampa Kid’s Foot Pain Center
Welcome to Advanced Podiatry Pediatric Foot Pain website! We have specifically designed this website for parents and their children who are seeking information about children’s foot pain, causes and treatments. There are major differences in pediatric foot pain versus adult foot pain, and they are not always treated in the same manner either! That’s why it is important for Advanced Podiatry to have a website dedicated to common foot and ankle problems that occur in kids, how to identify them at home, and when to seek treatment. We help relieve pediatric foot pain in a caring environment.
The podiatrists at Advanced Podiatry have helped alleviate foot pain in hundreds of kids and teenagers already at the Advanced Podiatry office, but there are still a lot of parents who haven’t been to our office asking, “What is normal growing pain versus abnormal pain in my child’s foot and ankle?” And that is exactly why we created this website! So you can review the information at the convenience of your home and make a knowledgeable decision as to whether your child should come see us.
It is common for children to be apprehensive to talk to their parents about foot or ankle pain that they may be experiencing. Usually parents may notice their child walking funny, not wearing certain shoes, quitting activities he or she used to enjoy, or purposely hiding their feet so you don’t see them. These are all common signs of discomfort that parents notice and discuss with us during their child’s first visit to Advanced Podiatry.
Whether your child is 3 years old or a teenager, our doctors at Advanced Podiatry help make them feel comfortable offering a friendly atmosphere and sense of humor. Our staff has also been trained to help children feel more comfortable in the exam rooms and calm down any fears or apprehension about coming to the doctor’s office.
Whether you’re worried about your child’s ingrown toenail or heel pain from playing sports, no concern is ever considered too small at Advanced Podiatry. So please feel free to browse through our website and call the office for an appointment with the doctors at Advanced Podiatry to help your child have pain-free, happy feet!

Total Ankle Replacement
Total Ankle replacements (TAR) are on the cutting edge of surgical technology. Painful ankles secondary to rheumatoid, post traumatic, or degenerative arthritis are all possible candidates for a TAR. Traditionally, ankle fusions were the only option for painful ankle joint arthritis. Ankle fusions are reproducible procedures that work well but are not without their drawbacks. Long recovery times, adjacent joint arthritis and the inability to move the ankle are the most common complaints associated with ankle fusions. TARs provide patients with the ability to have their ankle arthritis removed and the ankle joint resurfaced. During the procedure, both the top and bottom portions of the ankle are replaced by a sturdy press fit metallic covering with a polymer that floats in between, recreating the ankle joints natural range of motion. This allows patients to maintain their range of motion at the ankle joint and recover much faster than with an ankle fusion. Additionally, because of the maintained motion at the ankle joint, adjacent joints are not adversely affected, and therefore the risk of needing addition fusions is minimized. Dr. Bret Brennan is one of only a handful of surgeons in southwest Florida certified to perform a TAR. During your consultation he will discuss with you the positive and negatives of both TAR and ankle fusions and make an individualized decision about what will be best for you!

Minimally Invasive Foot Surgery
Foot surgery is traditionally done as “open surgery,” meaning the area being operated on has a long incision to allow the surgeon to view and fix the anatomy. In addition, “open surgery” typically involves more tissue disruption resulting in scar tissue and the potential for longer healing. Recently, updated instrumentation has allowed more foot and ankle conditions to be treated with minimally invasive foot surgery techniques.
Because minimally invasive foot surgery (MIFS), does not involve a long incision, it avoids significant damage to the muscles and soft tissue surrounding the foot. In most cases, this results in less pain after foot surgery and a faster recovery.
Foot surgery is recommended only when a period of conservative treatment — such as medications, orthotics, injections, offloading, and physical therapy — has not relieved the symptoms caused by your foot problem. Once the problem is identified the surgeon will inform you of all the surgical options that can be used to fix your foot problem. If you are a candidate for minimally invasive foot surgery then those options will be explained thoroughly.
Minimally invasive foot surgery techniques are beginning to be used for a wider range of foot procedures, and have been used for common procedures like a bunionectomy and hammertoe surgery since the early 1980s. A bunionectomy relieves strain put on the foot by cutting and removing portions of bone and realigning it for better function.
MIFS bunion surgery corrects problems with the big toe joint to alleviate pain that is experienced in a closed shoe and while walking. This educational material focuses on bunion correction utilizing minimally invasive techniques.
Description
Traditional bunion surgery requires the surgeon to make at least a 5cm opening in the skin and dissection of the soft tissue down to bone. With the soft tissue pulled aside and out of the way the surgeon can then see the boney problem needing correction. The surgeon then cuts the bone, moves it over, and places a screw across the bone cut to hold it in place. The surgeon moves the bone over so that the alignment of the bunion can be straight. Then the surgeon will typically close all the layers of soft tissue using absorbable stitches and finally sutures the skin closed with a skin stitch.
The bigger the dissection the more the soft tissue will be inflamed after surgery. This inflammation can result in nerve pain, decreased range of motion, and scar tissue. When the skin and soft tissue is pulled back during foot surgery the surgeon is able to identify anatomy easily. But the pulling back on the soft tissue often causes damage and swelling during the recovery period for the patient. This frequently leads to a longer and more painful recovery period.
Since minimally invasive foot surgery requires very small openings in the skin one can, correctly, conclude that the soft tissue damage is less extensive and therefore the pain and swelling after surgery is greatly lessened. This is very appealing to most of our patients since they desire very little downtime and do not want any inhibition of their lifestyle after surgery. Although minimally invasive foot surgery can be applied to many different foot conditions, it is important to note that not all patients will qualify for the surgery. It is not a solution for every type of foot problem but it does have its place when applicable.
Procedure
Minimally invasive foot surgery for a bunion is probably the most common application of this technique.
Anesthesia is achieved only through local injections of anesthetic around the surgical site and the numbness typically lasts between 4-6 hours. This is definitely the most unpleasant part of the procedure since the patient is awake and can feel the injections.
Thankfully, at Advanced Podiatry, our surgeons use cold spray and medications to make the patient more comfortable so that the injections cause minimal discomfort. The use of local anesthesia greatly reduces the risks to the patient because it eliminates the need for general or sedation anesthesia. After the patient’s foot is properly anesthetized the procedure can begin. For a typical bunion the first 2mm
opening in the skin will be made just adjacent to the bump. A tiny instrument will be used to create a portal to the bone. After the opening is made an instrument called a freer elevator is used to free up the soft tissue around the bone. Once this space is created a small MIS rotary burr is introduced into the portal and the bump is shaved off.

Regenerative Medicine
Advanced Podiatry is a leader in Regenerative Medicine in Tampa Bay for foot, ankle and Knee pain. He is one of the few podiatrists in the country that uses these amazing treatments. Advanced Podiatry has been using many of these protocols for about 8 years and has refined the techniques and combinations for the best outcomes.
At Advanced Podiatry we know that you are unique. Your problems and conditions are equally unique. That is why we have developed specialized treatment plans with customized natural ingredients designed just for YOU. The goal is for your body to heal naturally relieving pain, rebuilding and strengthening the structures of the foot and ankle.. The goal is to regenerate not degenerate! Traditional medicine places everyone in the same category and you become a number receiving a pill or surgery. We have the alternative for you.
With Regenerative Medicine we address the source of your problem to promote healing instead of treating your symptoms. We use custom treatments to rebuild your foot and ankle while promoting healing. Surgery and Cortisone lead to breakdown of tissue, Regenerative Medicine is the alternative for you.
What about Stem Cells? This treatment may be an excellent option in the future as it is refined. Currently, there is no evidence that this treatment is superior to all of the others available. Stem cell treatment is expensive and better outcomes are possible with many of the other options noted below. In addition, harvesting stem cells is invasive and more prone to complications.
All treatments are performed in the comfort of our office and do not require expensive surgery center or hospital fees.

Platelet-Rich Plasma – PRP
PRP or Platelet-Rich Plasma is a form of Regenerative Medicine that has been used at Advanced Podiatry for over 8 years. PRP treatments are used for healing of ligaments, tendons and joints of the foot, ankle and knee. In addition, wound healing can be accelerated with this treatment as well.
This specialized technique is safe because it uses your own blood cells and growth factors to heal your own body. Blood is safely drawn and a specialized process is used to prepare the platelet cells so that they are concentrated and vital without damage. This is a delicate process that is important for the best outcomes.
Platelets are blood cells that contain 800 different healing proteins and growth factors that support natural repair of your joints, ligaments and tendons by attracting your own body’s stem cells. Within the first hour after treatment, large amounts of these healing molecules are released and studies have shown that they continue to release for up to 7 days to help repair your foot, ankle and knee. All of these growth factors send signals to the body to activate the healing by attracting new strong tendon, ligament and cartilage cells leading to natural regeneration of your damaged tissues. In addition, these cells have strong anti-inflammatory effects.

Shockwave EPAT Therapy for Heel Pain
Shockwave, or EPAT, therapy is used for heel pain and other painful foot and ankle problems. Shockwave therapy stimulates tissue to encourage the body to start the normal tissue healing process. Additionally, shockwaves help to over-stimulate pain nerves, which can lead to decreased pain and sensitivity.
At Advanced Podiatry we perform EPAT, a non-invasive myofascial therapy to heal tendons, ligaments, muscles and connective tissue.
EPAT can be incorporated into your overall treatment plan with excellent outcomes. This therapy causes changes in the tissue of your foot and ankle and increases blood flow, reduces pain and relieves the tension at muscle trigger points to give you relief. Your body will respond naturally and bring in healing cells to give you long-term relief from pain and inflammation. These new cells actually will heal the plantar fascia and Achilles by laying down new stronger tissue at the attachments to the bone. So this is truly a form of regenerative medicine unlike cortisone which breaks down tissue with overuse.
This time tested treatment has been used for millions of people across the world with great success. The most common uses for this treatment are with plantar fasciitis and Achilles tendonitis. When these areas become injured and inflamed, the body goes into a protective state and the healing process often stops or slows significantly. The injury becomes chronic or long standing. EPAT therapy stimulates the area to enter the active healing state by encouraging healing cells to return and regenerate the fascia and tendon while removing to “bad swelling”.
Heel Pain
Elimination of pain symptoms can lead to rapid pain relief and increased weight bearing and activity on the affected foot.
The D-Actor 50 with EPAT (Extracorporeal Pulse Activation Technology) provides the most advanced non-invasive, in-office treatment solution that requires no numbing or anesthetics. The system delivers pneumatic pressure waves from outside the body to the area of discomfort using Biofeedback and a “point and shoot” method. The treatment triggers the body’s own repair mechanisms to initiate natural healing.
Call Advanced Podiatry today for a consultation to determine which regenerative medicine treatment is best for you, (813) 875-0555.

Prolotherapy
Prolotherapy is categorized in the medical field as Regenerative Medicine . This specialty medicine is exactly what it sounds like: promoting regeneration of tissue. It involves the use of natural substances that stimulate your body to heal an injury rather than using surgery to repair or replace your injured ligaments or joints. This growing medical spe cialty has been around since the 1950’s and has a huge fan-base with professional athletes since it is agreat alternative to surgery and requires little to no down-time.
Advanced Podiatry is one of the few offices in the Tampa Bay area which offers this service to patients and has been using Prolotherapy for over 7 years.
Prolotherapy is a treatment offered at Advanced Podiatry and involves a series of specialized injections which stimulate an inflammatory response initiating healing in an area of injury. You may suffer from plantar fasciitis, ankle ligament, tendon injuries or arthritic joints and take medications such as Advil or have attempted steroid injections which may only provide temporary relief. While most patients show positive responses to these standard conservative treatment options, some patients continue to have pain or repeat injuries. When these options fail, most surgeons recommend surgery as the final treatment option.
At Advanced Podiatry we believe Prolotherapy is an excellent alternative to steroid injections and surgery! Although surgical services are provided by our doctors, we understand the reluctance that you may have about surgery. Most surgeries require down-time including time off work, and surgery always has a risk of complications afterwards. During your initial consultation, the doctor will perform an exam and evaluate standard conservative treatment options prior to discussion of Prolotherapy, since it is most often used after initial conservative efforts have provided minimal relief. However, Prolotherapy is often used as a first line treatment for those patients who would like to avoid steroids. The treatments are often used in combination with musculoskeletal laser and custom arch supports or orthotics.
The treatments are performed in the office. No sedation or anesthesia is required except for use of cold spray and a small amount of local anesthesia to numb the skin in the area of the injection. The Prolotherapy injection consists of a specially prepared mixture of all natural substances including dextrose and an anesthetic solution that is injected into the area of interest (i.e. plantar fascia, ligament or joint). To enhance accuracy, ultrasound is used to visualize guidance of the injection right into the area of interest. Once the Prolotherapy solution has been injected, the body will begin to create an inflammatory response to the solution and sends growth factors and other cells that promote healing to the area. The new tissue laid down in the previously injured area produces increased stability and strength to the tendon, joint or ligament. Patients may have discomfort 1-3 days after the injection. Depending on the area being injected patients are sometimes placed in a walking boot to provide stability and decrease motion at the injection area for optimal healing; however, other times Prolotherapy works best when the area is allowed to move and enhances the healing cascade. Your doctor will discuss which is best for you during your consultation. Most patients will have an injection 1-2 weeks apart for a total of 4-6 injections.
Below is a list of foot and ankle problems that have shown great response to Prolotherapy. If you suffer from chronic pain in your foot or ankle and have not responded well to conservative treatment or are reluctant to have steroid injections or surgery, you are likely excellent candidate for Prolotherapy. Almost any type of injury, tendinitis or arthritic pain can be improved with its use, and there are rarely any contraindications, adverse effects or complications.
